Important: If you already have a State of Georgia Massage Therapist License, you are not required to get a work permit.

Fee: $50 per year

Required Documents:

• All Applicants: All applicants must provide a copy of their current business license.
• United States Citizens: Applicants must provide a valid government issued photo ID to establish their identity.

• Non-Citizens: Applicants must provide a valid government issued photo ID to establish their identity and proof that they are authorized to work in the United States.

Application Process:

1. Complete and submit an Work Permit Application.
2. Once your application has been reviewed, you will receive an email with a link and instructions to schedule an appointment.
3. Payment, a picture, and fingerprints will take place during your appointment.
